Hospital2Home

Constantly moving between hospital and home can be enormously disruptive and distressing for patients being treated for cancer. That’s why The Royal Marsden developed Hospital2Home (H2H), a scheme which has been so successful it is now being rolled out across London.

The scheme gives patients in palliative care more confidence about choosing to be cared for at home, by improving communication between hospital and community services.

H2H will be supported by a specialist team which will be funded by The Royal Marsden Cancer Campaign and based at the hospital. This team will make sure that patients choosing to move from hospital to home can be sure that their local healthcare services will be ready and waiting.
